Case management is the coordination of services for clients by allocating a professional to be responsible for the assessment of need and implementation of rehabilitation plans. It is usually required for clients who need ongoing support with their medical, vocational and psychological needs.

The underlying tasks of case management include:

  • rehabilitation planning
  • implementation
  • regular review
